Animal Blood Donor Database

1. Terms of Participation
You agree to include information about yourself and your pet in Armenia’s first animal blood donor database, established by the veterinary clinic “Shunn u Katun” (LLC “Vetmir”).
To participate in the project, your pet must meet the following criteria:
— Aged between 1 and 8 years
— Weight: from 20 kg (for dogs) or from 3 kg (for cats)
— Good overall health
— Up-to-date vaccinations (including for infectious diseases and rabies)
— No chronic illnesses
— Has not received medical treatment or taken medications in the past 2 months
You also confirm that:
— Participation is voluntary, and you have the right to withdraw at any time
— You understand that all financial arrangements with the recipient’s owner (expense reimbursement, potential compensation) are handled directly between the parties — without the involvement of the clinic or the database operator
— The transfusion procedure may be performed not only at “Shunn u Katun” but also at any other veterinary clinic in Armenia where the recipient is being treated
— You agree to provide accurate and truthful information about yourself and your pet
2. Consent to the Processing of Personal Data
You consent to the collection, storage, processing, and use of the following personal data:
— Full name
— Contact phone number and email
— City of residence
— Information about your pet (species, breed, age, weight, health status)
— Other information voluntarily provided in the form
Your data will be used exclusively for the purposes of the project, including:
— Matching a suitable donor or recipient
— Contacting you regarding participation
— Maintaining an internal donor database
You agree that your data may be shared with:
— Staff of the “Shunn u Katun” clinic
— The operator of the donor database
— The recipient’s owner, in case your participation in a specific donation is approved
3. Additional Consents
You also:
— Consent to having your contact details added to the project’s Telegram and WhatsApp groups (if launched), where recipient requests are published
— Permit contact via phone, messengers, and SMS
— Agree to be contacted by the database operator or recipient’s owner regarding potential donations
4. Withdrawal of Consent
You may withdraw your consent and request that your data be removed from the database at any time by contacting the “Shunn u Katun” clinic using the contact information on the website.

By filling out the request form on this page and clicking the “Request a Donor” button, you confirm that:
1. Terms of Participation
You voluntarily join the social project “Animal Blood Donor Database”, implemented by the veterinary clinic “Shunn u Katun” (LLC “Vetmir”).
You confirm that the animal specified in the request may require donor blood and that you are responsible for the accuracy of the information provided.
Participation in the project is free of charge. However, all expenses related to the donation process (examinations, blood collection, transportation, etc.) must be compensated by the recipient’s owner.
In some cases, these expenses may be partially or fully covered by sponsors or charitable fundraising, if such support is available.
All financial arrangements (including possible compensation to the donor) are handled directly between the parties — without the involvement of the clinic or the database operator.
The transfusion procedure can be performed either at the “Shunn u Katun” clinic or at any other veterinary clinic in Armenia, as agreed between the parties.
You acknowledge that we do not guarantee the availability of a suitable donor.
2. Consent to the Processing of Personal Data
You consent to the collection, processing, storage, and use of your personal data, including:

• Full name
• Contact phone number and email
• Location (city)
• Information about your pet (species, breed, age, weight, diagnosis, condition, etc.)

Purposes of data processing:
— To match a suitable donor for your pet
— To share information with potential donors
— To publish the request in the project's open channels (Telegram, WhatsApp)
— To contact you regarding donation-related matters
You confirm that:
— You agree to share the specified data with potential donors and the database operator
— You agree to the publication of limited request information in the project’s open channels to increase the chances of a response (including pet data, city, and phone number)
— You consent to be contacted by phone, messengers, including via SMS and calls
— You are informed of your right to withdraw this consent at any time and request the deletion of your data from the database

— You voluntarily agree to participate
— You agree to the conditions for the processing of personal data
